1. Birth certificate (both applicats)
2. CENOMAR(both applicants)
3. Valid ID (both applicants)
4. Valid ID of two(2) witnesses
5. Joint Affidavit of cohabitation
*Deadline of application: June 10, 2022
For Application for Marriage license:
1. Birth Certificate of both appicants
2. CENOMAR of both applicants
3. 2×2 picture of both applicants
4. Valid ID of both applicants
5. Parents’ Consent/advise(18 – 24 years old)
6. Pre-Marriage Orientation Counseling
Certificate
7. Death Certificate of spouse if widow or
widower
8. Certificate of Legal Capacity if not a
Filipino Citizen
10. Judicial decree of nullity/annulment of
Marriage.
