Requirements for Application for Marriage License:
• Birth Certificate – Both (Male & Female)
• CENOMAR – Both (Male & Female)
• 2×2 Picture – Both (Male & Female)
• Valid ID – Both (Male & Female)
• Parents’ Consent/Advise (18-24 years old)
• Pre-Marriage Orientation and Counseling
• RHU-I (Every Monday 8:00am-12:00nn)
• DSWD (Every Monday 1:00pm-5;00pm 18-24 yrs.old)
• Church /Chapel – Solemnizing Officers (Pre-marriage Counseling)
• Death Certificate of Spouse if (widow/widowers)
• Certificate of Legal Capacity(if not Filipino Citizen)
• Divorce papers if divorce/ Judicial decree of annulment/nullity of marriage

• Birth Certificate – Both (Male & Female)
• CENOMAR – Both (Male & Female)
• Valid ID – Both (Male & Female)
• Valid ID of two (2) witnesses
• Joint affidavit of Cohabitation
